Steps to register your child for school in Derby:
Complete the online registration by clicking the appropriate online student registration link in the box below and fill out the complete information for each child you are registering.
Collect all necessary documentation detailed on the checklist below.
Once you have completed the above two items, you should choose one of the following options:
You may scan and email all documents to registration@derbyps.org. Your email should include contact information including a phone number where you can be reached. (Do not forget to include the student's name, grade and the school they will be attending in the subject line of your email).
If you prefer to call, please dial 203-736-5027, option 9 Central Office then press option 1 Student Registration to schedule a telephone conference to review the online registration and the completion of your documents.
**No registration is complete without all of the required documentation.**

REGISTRATION DOCUMENTATION CHECKLIST
Documentation listed below is required by the Derby Public School Board of Education in order to register your child. Please use this checklist to track the documents you need to collect. A parent or legal guardian is required to register student(s).
The child's original full-size birth certificate;
A Health Assessment Form that includes your child’s immunization record signed by a doctor showing an examination within the last 12 months. All students must have the appropriate vaccinations to enter school. (Age 5 and above is a Blue form and Age 4 and below is a Yellow Form.)
Proof of Residency (All of the below items are required)
Mortgage statement: If you own the house please print out your statement.
Current Lease and Landlord/Property form: If you rent your home you must also provide a notarized Landlord/Property Owner form Click Here: "for Landlord/Property owner certification of Residency Form". We cannot proceed the registration until this is complete and notarized.
Two Utility Bills: showing your name and Derby address (for example, a cell phone, utility, Auto Insurance or Registration)
Valid Identification of Parent or Guardian: (CT Driver’s License or ID with Derby address)
Legal Custody Documentation, if applicable.
Special Education Records IEP or 504 Plan, if applicable.
Students entering high school should provide their most recent transcripts to facilitate creation of their schedule. Without this information, the student's start date may be delayed.

LITTLE RAIDERS UNIVERSITY
Registration for Little Raiders University is on a first-come, first-serve basis.
Spots are limited. You need to complete the online registration and have all necessary documentation from the list above to complete the process.
Placement is not guaranteed, as the number of available openings is subject to change.
You will be notified in July by phone, if you received a spot. And letters will be sent later in the month.
